Trauma Lab is a free anatomy and physics sandbox game by Abynotic Studios, released on itch.io on October 21, 2026. You can play it online right here in your browser, or download the official build for Windows or as an Android APK from the developer's itch.io page. Every bullet is tracked through skin, muscle, bone and organ, live vitals react in real time, and a challenge mode asks you to keep the patient alive with medicine alone. This fan page explains how to play, where to get the safe download, and how to beat all 10 emergencies.

What Is the Trauma Lab Game?

Trauma Lab describes itself as "an anatomy and physics sandbox." You get a single test subject in a lab and a huge set of tools: shoot, stab, burn or poison him, then look under the skin to see exactly what happened — or put the weapons down and try to save him.

What makes the Trauma Lab game different from a typical ragdoll sandbox is how deep the simulation goes:

  • Ballistics that follow the bullet. Every shot is traced through what it hits, from a fractured rib to a perforated lung to a severed aorta. Ammo types include FMJ, hollow point, AP and fragmenting rounds.
  • X-ray layers. Peel back skin, muscle, organs, arteries and veins, bones and nerves to inspect the damage. The X-ray view dims the room so organs and bullet paths stand out.
  • Live vitals. Heart rate, blood pressure, SpO₂ and blood loss update in real time on the patient monitor. Shock, cardiac arrest and bleed-out all play out on screen.
  • Medicine and surgery. Bandages, tourniquets, drugs and chemistry, plus a Simple or Complex surgery mode with stitches, bone plates, suction and blood top-ups.
  • Powers. Lightning, fireball, freeze, acid, black hole and more.
  • Scenes. The default test chamber, plus a trauma bay, field hospital, castle and battlefield, each with its own lighting.

The game runs on the developer's own HTML5 engine, so it works in a desktop or phone browser. It is still in development and updates arrive very quickly — it went from v1.06 at launch to v1.19 within two days. The developer lists AI assistance for code, graphics and sounds on the itch.io page.

How to Play Online

To play online, press Play above. The browser version is the same build that runs on itch.io, so there is nothing to install. Use the Fullscreen button (or F11 on PC) for the best view. On a phone, the game switches to a dedicated phone layout with bigger vitals and pinch-to-zoom.

Default PC controls (you can rebind them in Options):

Tips: if the game lags, lower the gore level in the Options menu — that is the developer's own advice. If you want it bloodier, the developer recommends the extreme gore setting instead. There is also a built-in Russian language option that switches on automatically when your browser is set to Russian.

Trauma Lab Download: Windows, Browser and Android APK

How to download from itch.io

  1. Open the official game page on itch.io.
  2. Click Download Now.
  3. Enter an amount, or choose "No thanks, just take me to the downloads" to get it free.
  4. Pick your file.

The Trauma Lab APK is small (about 1 MB). To install it:

  1. Download TraumaLab-Android-…apk from the itch.io page in your phone's browser.
  2. Open the file. Android will ask you to allow installs from this source (your browser or file manager) — allow it.
  3. Tap Install, then open the game.

"Windows protected your PC" warning

Windows SmartScreen may show "Windows protected your PC" when you open a new indie download. The developer explains that this happens because the game is new and hasn't been downloaded by many people yet — there is no virus in the files. Click More info → Run anyway. Only do this with files downloaded from the official itch.io page.

Game Modes: Sandbox and Keep Him Alive

  • Sandbox — free play with every weapon, tool, chemical and power. Experiment, inspect wounds with X-ray and watch the vitals react.
  • Keep Him Alive — a challenge mode with 10 real emergencies that you treat with the Medicine tab only. Press H to open it.
  • Story mode — in development. The latest patch notes describe a three-room prototype with guards, cover and a sentry gun. New Story builds go to supporters and Discord playtesters first; the free build shows a teaser.

Keep Him Alive: All 10 Emergencies (Walkthrough)

Keep Him Alive uses the same physiology as the sandbox: bleeding, drugs, heart and lungs behave exactly the same. You win once the patient has been stable for 15 seconds in a row — bleeding controlled, blood pressure and oxygen holding. You lose if he dies, or if his systolic pressure stays under 60 mmHg for 6 seconds.

The table below lists every case in Trauma Lab v1.19 with the in-game hint.

#EmergencyPatientWhat's wrongHow to save him (in-game tip)
1Gunshot · thighSoldier, 249 mm round through the left thigh; femoral artery torn (up to ~1 L/min blood loss)Tourniquet high on the thigh, pack the hole with gauze, then give blood. TXA (tranexamic acid) helps reduce bleeding deaths.
2Sword cut · armSquire, 19Brachial artery cut in the inner upper armTourniquet above the cut, then suture or clamp the vessel (Surgery). Replace lost blood.
3Open fracture · shinMotorcyclist, 31Tibia through the skin, shin artery torn, severe painTourniquet above the knee, gauze and bandage, a bone plate (Ortho), morphine or ketamine for pain.
4Heart attack · cardiac arrestMan, 72Blocked coronary artery; rhythm collapses into VFDefibrillate (VF is shockable). Adrenaline every few minutes; amiodarone after the 3rd shock keeps the rhythm stable.
5Heart wall rupture · needs surgeryWoman, 78Dead heart muscle tore; blood leaking around the heartFluids and blood to buy time, then chest surgery to repair the heart. Defib if it fibrillates.
6Opioid overdoseMan, 29, found unresponsiveFentanyl stopped his breathing; pinpoint pupilsNaloxone reverses it within a minute — but it wears off sooner than fentanyl, so watch for him going under again.
7Sleeping pills + alcoholWoman, 45Benzodiazepine overdose on top of alcoholFlumazenil reverses the benzodiazepine (can trigger seizures in long-term users). Alcohol has no antidote: wait it out.
8House fire · cyanideFirefighter, 38Hydrogen cyanide from burning plastics; SpO₂ looks fine but cells can't use oxygenCyanide antidote (hydroxocobalamin), plus fluids for blood pressure.
9Snake biteRanger, 34Viper venom stops clotting and attacks vesselsAntivenom — but one dose too many can trigger anaphylaxis, so keep adrenaline ready.
10Bee sting · anaphylaxisHiker, 26Airway swelling shut, vessels dilatingAdrenaline, then fluids for the pressure.

General tips for Keep Him Alive: stop arterial bleeding first (bright red, pulsing), then replace blood, then deal with pain or the poison. Keep an eye on systolic pressure — falling under 60 mmHg is the fastest way to fail.

Medical disclaimer: this is a game. The tips describe in-game mechanics and are not medical advice.

Medicine, Drugs and Serums

The Medicine tab includes these drugs (doses as shown in the game): Adrenaline 1 mg, Amiodarone 300 mg, Atropine 1 mg, Fentanyl 100 mcg, Flumazenil 0.5 mg, Ketamine 50 mg, Midazolam 5 mg, Morphine 10 mg, Naloxone 0.4 mg, Paracetamol 1 g and Tranexamic acid 1 g, alongside tools like tourniquets, gauze, bandages, a defibrillator, fluids and blood.

Sandbox also has fantasy serums:

  • Super serum — heals everything still inside the body (organs, brainstem, bones, wounds) and closes torn skin and muscle. It does not revive him and does not regrow limbs, as the developer confirmed in the comments.
  • Ultra serum — revives and regrows: lost limbs and organs grow back, and organs that fell out are drawn back in.
  • Super-soldier serum — also available in Sandbox.
  • Inhibitor — stops everything: every drug, serum and poison.

Want the patient to sleep? The developer's answer: use ketamine.

Weapons, Powers and Tools

Weapons in Trauma Lab v1.19 include a pistol, revolver, SMG, rifle, assault rifle, shotgun, sniper rifle, crossbow, knife, machete, axe, baseball bat, grenade and C4, with several ammo types (FMJ, hollow point, AP, fragmenting). Latest Update: v1.19

The game ships with full patch notes inside it (menu → Patch notes). Highlights:
